Large meta-analyses covering tens of thousands of implants consistently report success rates around 97% to 98% at five years and 94% to 96% at ten years. This means that out of 100 implants placed, two or three will have failed within five years, and another two or three will fail between years five and ten. Early failures happen in the first 6 months and are usually due to failed osseointegration — the bone simply did not grow onto the implant. Causes include overheating during drilling, contamination, inadequate primary stability, smoking and uncontrolled diabetes. Late failures, occurring years after successful integration, are almost always caused by peri-implantitis — bacterial inflammation of the surrounding tissues, often linked to poor hygiene, excessive cement residue or occlusal overload.
