When is it safe to extract a tooth and place an implant in the same visit? Immediate implant protocols, success rates and aesthetic advantages explained.
A tooth is failing. Your dentist has confirmed it needs to come out. Now comes the difficult part: do you extract it, heal for three months, and then go back for an implant — or can you have both done in a single afternoon? This is the immediate implant question and the right answer depends on three things: the reason for extraction, the shape of the socket and the quality of surrounding bone.
Two decades ago the standard was always staged: extract, wait 3-6 months for the socket to fill in, then place the implant. Modern research showed that waiting often caused significant bone resorption — up to 50% width loss in the first 12 months — making eventual implant placement harder and sometimes requiring grafting. Immediate placement preserves the socket architecture, reduces overall treatment time and usually gives better aesthetic results, especially in the smile zone.

For years, an infected tooth was considered an absolute contraindication for immediate implant placement. Recent studies have softened this view: with aggressive socket debridement, antibiotic coverage and careful primary stability, implants placed into formerly infected sockets achieve success rates close to those placed in healthy sites.
